If you’ve researched laser skin resurfacing, you’ve probably seen dramatic CO2 laser before-and-afters—along with equally dramatic photos of the recovery.

But does more downtime automatically mean better results?

Not necessarily.

Both Erbium and CO2 lasers can improve signs of aging and skin damage through resurfacing and collagen remodeling, but there are important differences in treatment intensity, heat, and recovery.

What’s the Biggest Difference Between Erbium and CO2?

One of the biggest practical differences I explain to patients is the amount of heat transferred into the skin. I like to think of Erbium as very precise resurfacing with less heat, while CO2 laser resurfacing provides more heat-driven tightening.

You can still get collagen remodeling and skin tightening with Erbium, although it can be a little less drastic than an aggressive CO2 treatment. The tradeoff is that Erbium allows us to be less aggressive and potentially give patients less downtime.

That flexibility is important because not everyone needs the most aggressive laser treatment available. Someone wanting brighter skin, smoother texture and improvement in very fine lines doesn't necessarily need the same treatment intensity as someone with deeper wrinkles, significant photoaging or acne scarring.

Can Erbium Laser Give Results Like CO2?

CO2 has earned its reputation for producing beautiful results. In fact, we see patients all the time who come in thinking they need CO2 because they’ve seen the dramatic post-treatment photos followed by spectacular before-and-afters.

I love how beautiful CO2 results can be. But many patients don't realize that we can also achieve beautiful resurfacing results with an ablative Erbium treatment.

That was another reason I chose Erbium for Skin Treats. We can get meaningful skin resurfacing, collagen remodeling and tightening without automatically committing every patient to the intensity and recovery associated with a more aggressive CO2 treatment.

Proper education and guidance go a long way in determining what your skin actually needs, rather than simply choosing the most aggressive treatment.

Who Is a Good Candidate for Erbium Laser Resurfacing?

Some of my favorite Erbium patients are those who already use great skincare and have invested in collagen-stimulating treatments like microneedling but are ready for more significant resurfacing.

I also love treating older patients with significant photoaging because we can see such a beautiful improvement.

Erbium can be used to address concerns including:

Fine lines and wrinkles
Sun damage and photoaging
Rough or uneven texture
Perioral lines around the mouth
Enlarged pores
Acne scarring

Perioral lines are one concern I particularly like treating because they can often be softened with ablative Erbium resurfacing.

For acne scarring, we may also incorporate Erbium after treatments such as Pico laser and RF microneedling. Different technologies can play different roles in an individualized acne-scar treatment plan.

How Do I Decide How Aggressive to Go With Erbium?

One of the great things about Erbium is that the treatment can be customized to your skin and your goals. Not everyone needs—or wants—the most aggressive treatment.

When deciding how deeply to treat, I look at several things:

Your skin type
Amount of sun damage
Depth of fine lines and wrinkles
Acne scarring
Pigmentation concerns
How your skin responds to inflammation
How much downtime you’re comfortable with

Someone looking for a brighter complexion, smoother texture or improvement in very fine lines may benefit from a lighter treatment with less downtime.

Deeper wrinkles, more significant sun damage or acne scarring may require a more aggressive treatment and longer recovery.

I like to describe Erbium resurfacing as polishing a penny. The goal isn't simply to turn the laser as high as possible. It's to determine the appropriate amount of resurfacing for the skin in front of me.

Does Skin Type Matter With Erbium Laser?

Absolutely...

Erbium works especially well for our lighter Fitzpatrick skin types. We have to be much more conservative as skin becomes darker because these patients can react more aggressively and may have a greater risk of developing post-inflammatory hyperpigmentation (PIH).

I also consider how your skin responds to inflammation. If you’re prone to developing dark spots after irritation or have a history of melasma, I may choose a gentler approach to reduce the risk of unwanted pigmentation.

This is one of the reasons an individualized skin assessment matters so much with laser resurfacing. The question isn't simply how aggressive can we go? It’s how aggressive should we go for your skin?

Erbium vs. CO2: Which Laser Is Better?

Neither laser is universally better.

CO2 can produce beautiful results and may be appropriate for someone who needs—and is prepared for—a more aggressive treatment and recovery.

Erbium gives me the ability to provide meaningful resurfacing while customizing treatment intensity based on the patient’s concerns, skin type and desired downtime. For my patients at Skin Treats, that versatility is one of the biggest reasons I chose it.
